The Royal Pharmaceutical Society is developing a baseline standard for a patient-centred Pharmaceutical Care Record to supplement and expand upon the data already held by our patient medication record (PMR) systems. Pharmacists have a key role to play in determining the requirements/standards for the medicines & medicines management elements of records.
At a time when pharmacists are seeking to establish new professional roles and services, there is an urgent need to develop an information infrastructure to support a pharmaceutical care strategy across the profession. Moreover a standard content and format for a pharmaceutical care record would support the development of quality metrics to enable commissioning and monitoring of innovative pharmacy services.
